- Please bring your picture ID and all previous immunization records for each individual needing services to your appointment
- Prior records must be available prior to or at visit time
Immunization Record Requests
DOH-Miami-Dade only has access to immunization records that have been entered into the Florida Shots database by private physicians, pharmacies, or the Florida Department of Health.
- If you want us to check if your record is in Florida Shots please complete an Authorization to Disclose Form and fax it to 786-845-0598 with a copy of a valid state ID or passport.
- If your record is in Florida Shots, we will be able to fax it back to you. Please allow up to 5 business days to process your request.
- If you do not have access to a fax machine, you can request an appointment to receive your record at one of our clinics.
Adult & Travel Immunizations
Immunizations are available to adults interested in receiving routine vaccines and others that may be considered for international travel destinations.

This is the approximate cost of the immunization. All prices and vaccine availability are subject to change. If more than one immunization is requested per visit, a discount of $ 6.13 is applied for each additional immunization.
