If you are an employer requesting the cost of immunization services your employees, please email us your request along with your contact information.


Immunization Services Program at the Florida Department of Health in Miami-Dade (DOH-Miami-Dade) was established in 1963 as the local representative of the state of Florida's Bureau of Immunization.

Goals

  • Control/eliminate vaccine preventable diseases for residents and visitors
  • Provide barrier-free immunizations and education for infants, children and adults
  • Provide education to parents or guardians on the importance of vaccines
  • Educate and monitor private and public sectors and provide consultative screenings of immunization records along with vaccine handling and storage techniques
  • Provide ongoing surveillance of vaccine preventable diseases.

Immunization Services

Immunization Record Requests

DOH-Miami-Dade only has access to immunization records that have been entered into the Florida Shots database by private physicians, pharmacies, or the Florida Department of Health.

If you want us to check if your record is in Florida Shots please complete an Authorization to Disclose Form and fax it to 786-845-0598 with a copy of a valid state ID or passport.

If your record is in Florida Shots, we will be able to fax it back to you. Please allow up to 5 business days to process your request.

If you do not have access to a Fax, request an appointment to receive your record at one of our clinics.


Free Immunizations for Children (excluding travel vaccines)

The Florida Department of Health in Miami-Dade provides free immunizations to all children that visit our clinics, based of the Vaccines for Children (VFC) Program. VFC is a public/private partnership that is federally-funded, and administered by the state of Florida. 

Since 1994, VFC has distributed over $100 million in vaccines to Florida private physicians, hospitals, and other public and private health care providers to reduce cost as a barrier to immunizing children. DOH-Miami-Dade ensures that every child is given access to immunizations regardless of financial status. 

The vaccines that are offered free of charge for children, ages 0 through 18 are DT, DTAP, TD, TDAP, Hepatitis A and B, HIB, HPV-9, Influenza, IPV, MCV4, Meningococcal B, MMR, PCV13, Rotavirus and VZV.

If you would like to expedite your child’s clinic visit, fill out pre-registration forms (Travel and Adult, as well as Pediatric, one per child), and bring them with you to the scheduled appointment.


Daycare Immunization Team

The Florida Department of Health in Miami-Dade’s immunization program collaborates with the Department of Children and Families in auditing daycare immunization records. This collaboration was created to improve immunization rates in daycares and prevent outbreaks of vaccine preventable diseases.

The team visits daycares and reviews all immunization records and assist daycares in immunization compliance. Children whose records are out of compliance will be provided a letter for the parents, advising that they are due for immunizations or that their immunizations are out of compliance. 


Adult Immunizations

Our immunization program offers adult vaccines to support health, and requirements for college/university and immigration.

If you would like to expedite your clinic visit, fill out the Travel and Adult Pre-Registration Forms and bring them with you to the scheduled appointment.

Vaccine Immunization Fee(s) Reference - New fees will be effective September 20, 2024Pricing for travel vaccines does not include the travel immunization prescription fee of $65.


Vaccine Storage and Handling

Requirements necessary to ensure proper vaccine storage and handling include appropriate refrigeration at specific temperatures. When recommended temperatures are not maintained, vaccines may lose their potency and result in the recipients being inadequately immunized.

Our staff performs on-site immunization reviews at VFC provider clinics to ensure that vaccines are stored properly.
