Comprehensive community health assessment and health improvement planning are the foundations for improving and promoting healthier communities.
The Community Health Assessment (CHA) consists of secondary data collected through the synthesis of existing data from national, state, and local sources which are analyzed to learn about health status, quality of life, and risk factors for poor health outcomes among residents of Miami-Dade County. This assessment is monitored and updated on an annual basis to ensure the data is being evaluated to track progress in our community.
The Community Health Improvement Plan (CHIP) is a 5-year plan for improving community health and quality of life. The CHIP sets out goals for the Miami-Dade County public health system, which includes a range of stakeholders, such as state and local government agencies, health care providers, employers, community groups, nonprofit organizations, universities and schools. The new 2025-2030 CHIP Priority Areas are: Access to Care, Behavioral and Mental Health, Chronic Disease Prevention, Maternal Child Health, and Healthy Weight/Physical Activity/Nutrition.
