Community Health Action Team (CHAT)

  • The Community Health Action Team (CHAT) is a mobile public health response outreach team in the Office of Community Health and Planning at the Florida Department of Health in Miami-Dade County. CHAT consists of Senior Community Health Nurses, and Health Services Representatives. CHAT staff members provide specialized public health services including prevention, education, advocacy, regulation, and direct services within Miami-Dade County. The overall goal of services and initiatives is to lower the incidence of chronic disease and obesity by raising awareness of proper nutrition and healthy lifestyles. Program services include:

    • Support health fairs and health promotion events
    • Biometric screenings: blood pressure, body mass index (BMI), body fat analysis (BFA), blood glucose and cholesterol, and carbon monoxide breath analysis (note: when offered all-together, this is called a Passport to Health event).
    • Healthy lifestyle education based on screening results
    • Referrals to primary healthcare providers, community agencies and other resources as needed
    • Health and wellness classes/presentations
    • Technical assistance for worksite wellness programs
    • Journey to Wellness Rx is a non-pharmaceutical prescription provided by healthcare personnel (nurses, health services representatives, health educators, etc.) to promote healthy lifestyles including proper nutrition, regular physical activity, and tobacco prevention/cessation for weight and chronic disease prevention and management.     Community Health Event Request (CHER) is an online form that serves as a formal request for the Florida Department of Health in Miami-Dade County and/or the Consortium for Healthier Miami-Dade to participate in a community or employee health event. It is recommended that requests be submitted 30 to 60 days prior to an event whenever possible, but at least 2-3 weeks beforehand. The link to the CHER form can be found HERE. The details of the community health events will be posted on the Consortium for a Healthier Miami-Dade website under Events - Community Calendar so that other Consortium community partners can participate if they wish to do so.
