School Entry Health Examination

Florida Statute 1003.22 (Chapter 1003 Section 22 - 2021 Florida Statutes - The Florida Senate (flsenate.gov)) requires that students in grades prekindergarten through twelfth grade initially entering a public or private school in the state present documentation of a physical examination performed within one year prior to enrollment (registration date). The school entry health examination (Form DH3040) must be completed and signed by a legally qualified professional licensed to perform physical exams in the United States.

The Florida Department of Health in Miami Dade’s immunization program provides requirements for school entry including School Entry Health Examination (Form DH3040) and the Florida Certification of Immunization (Form DH680).

As per CDC recommendations, certain people should be tested for TB infection because they are at higher risk for being infected with TB bacteria  (Who Should be Tested for TB Infection | TB | CDC) . It is possible that your children require a TB test as part of the TB assessment that’s implicit in the school entry health examination. Lab test results usually take 7 days to be reported. This is going to be the estimated time that you may need to wait until a completed and signed DH3040 form is provided. Parents or legal guardian will be contacted once the DH3040 form is available to be picked up.

Children do not need to be fasting at the time of the school entry health examination; the blood work that will be performed does not require it.

If your request is to have a school entry health examination and you will provide all the necessary lab work (Hb/Hct, lead, urinalysis, and TB test if it is required) from an accredited agency and no older than 6 months, the cost will be $35.00.

If the school entry health examination requires a TB test along with the necessary lab work (HB/Hct, lead, urinalysis), the cost will be $75.89.
