Florida requires certain vaccines to be administered before children may enroll and attend childcare and school.

Public/Non-public Preschool Entry
Public/Non-public Schools Kindergarten Through 12th Grade

Forms Required for Immunization Documentation:

• Department of Health Form 680
• Ask your doctor, clinic or the Florida Department of Health in Miami-Dade County for the Department of Health DH680 form. Don't forget to take your child's immunization record with you.
