College-University Health Forms

Colleges and universities require that students complete a student health record form. It is a prerequisite to enrollment in most of the college-university programs.

The Florida Health Department in Miami Dade’s immunization program provides the service to complete college-university forms. Call our appointment line at 786.845.0550 to schedule an appointment.

Once you showed up at your appointment, your college-university form will be individually assessed to determine the required services to complete it, including lab work. You will be provided with a lab order related to your specific requirements (see below for guidance) to visit one of the patient service centers from Quest to complete it. All related charges will be paid at your appointment, no additional charges will be applied when you visit the patient service center from Quest to complete the necessary lab work. Once the lab work results are available and the college-university form is completed and signed, you will be contacted to come to the clinic to pick it up.

If the service requested is college consultation ONLY. The cost will be $25.00. This will be the case in which the college-university student will bring all the required documentation (lab work and/or immunization history) from an accredited agency to have a legally qualified and licensed professional filling out the college-university form.

When your request is to have a college consultation with drug test. The cost will be $194.65. It will include MMR and VZV titers, Hepatitis B panel, QuantiFERON, Hb/Hct, urinalysis, and a 10-panel drug test.

If your request is based on MMR and VZV titers, Hepatitis B panel, QuantiFERON, Hb/Hct, and urinalysis without the drug test. The cost, in this case, will be $136.21.

You will be charged for the services at the clinic; however, you may be able to claim the lab tests to your insurance if it is covered. If your insurance pays for the services, a refund will be issued.

None of the cases described above include required vaccines. If your college-university form requires that you receive certain vaccines, please refer to the section that describes pricing for the adult vaccines.
