Strategic Plan

The DOH-Miami Dade County has agency-wide plans that provide guidance on the agencies vision, and community needs. These include the following:

Strategic Plan 2023-2027

The Community Health Improvement Plan (CHIP) is a five-year plan to improve community health and quality of life in Miami-Dade County. It is a long-term systematic effort to address the public health concerns of the community. The CHIP aligns with national and state public health practices using Healthy People 2020, Healthy People 2030, and the State Health Improvement Plan (SHIP) as a model. The plan identifies high-impact strategic issues and desired health and public health system outcomes to be achieved by the coordinated activities of the partners who provide input.

Community Health Improvement Plan 2019-2024

The Community Health Assessment (CHA) provides comprehensive information about our community’s current health status and needs through systematic, comprehensive data collection and analysis. The Mobilizing for Action through Planning and Partnerships (MAPP) process is used for the assessment, which is a community-driven framework for improving community health. The completion of a CHA supports the development of the Community Health Improvement Plan, which identifies how and where resources should be allocated to best meet community needs.
