  • Community Health Outreach Program (CHOP) – complete a CHOP form online to submit a formal request for the Florida Department of Health in Miami-Dade County, as well as the Consortium for a Healthier Miami-Dade and community partners, to participate in your wellness fair. It is recommended that a CHOP form be completed 30 to 60 days prior to an event whenever possible, but at least 2-3 weeks beforehand. Once a CHOP form is completed, someone from the Office of Community Health and Planning will be in contact to confirm our availability once our services are requested. Additionally, the details of the event will be posted on the Consortium for a Healthier Miami-Dade website under Events - Community Calendar so that other Consortium community partners can participate if they wish to do so.
