Miami-Dade County Well-being Survey

Miami, Fla.The Florida Department of Health in Miami-Dade County (DOH-Miami-Dade) invites you to complete the 2025 Miami-Dade County Well-being Survey. The survey is meant to identify the needs, opinions, and views of Miami-Dade County residents as it relates to the communities they reside. 

The survey is available in English, Spanish, and Creole and will remain open until the needed responses are received. 

The survey helps to answer the following questions:

  • What is important to the community?
  • How is the quality of life perceived in the community?
  • What assets does the community have that can be used to improve community health?

The Well-being Survey folds into the Mobilizing for Action through Planning and Partnerships (MAPP) model of community health improvement as one of the three types of assessments that informs the new Community Health Improvement Plan (CHIP). Taken together, the three assessments give a complete view of health and quality of life in Miami-Dade County and help make up the Miami-Dade County Community Health Assessment (CHA).
