DOH-Miami-Dade’s Vital Records Office Closed

MIAMI – The Florida Department of Health in Miami-Dade County (DOH-Miami-Dade) announces the temporary closure of its North Dade Vital Records Office located at 18680 NW 67th Avenue in Hialeah. The office will remain closed to the public until further notice.

Residents may visit the following locations: 

Health District Center
1350 NW 14th Street, Ste. 101
Miami, FL 33125
Monday thru Friday
8:00 a.m. – 4:00 p.m.

West Perrine Center
18255 Homestead Avenue
Miami, FL 33157
Monday thru Friday
8:00 a.m. – 4:30 p.m.
